Coverage and benefits vary based on your individual policy, deductible status, and plan requirements. As a courtesy, our team will verify your benefits prior to your first appointment whenever possible. Please note that insurance verification is not a guarantee of payment — final patient responsibility is determined once claims have been processed by your insurance carrier.

Understanding Your Benefits

Deductible

A deductible is the amount you are responsible for paying before your insurance begins contributing toward covered services. For example, if your deductible is $1,500 and you have only met $500, you may be responsible for additional treatment costs until the deductible is satisfied.

Co-Pay

A co-pay is a fixed amount required by your insurance plan and is typically collected at the time of service.

Co-Insurance

Co-insurance is the percentage of treatment costs you may be responsible for after your deductible has been met. For example, if your insurance covers 80% of treatment costs, you may be responsible for the remaining 20%.

Out-of-Pocket Maximum

Your out-of-pocket maximum is the highest amount you are required to pay during your policy period before your insurance begins covering eligible services at 100%.

Out-of-Network Benefits

Many patients choose AID Performance Physical Therapy for our individualized treatment approach, one-on-one care model, and focus on long-term results and performance. Depending on your insurance plan, you may still have out-of-network benefits available. Our team can help review your options and explain any potential reimbursement opportunities. For additional information, please reach out to your insurance company directly.

Payment Expectations & Policies

To provide a seamless experience for both our patients and clinical team, we ask that:

  • All co-pays and known balances are paid at the time of service
  • All patients maintain a valid form of payment securely stored on file while receiving care
  • Patients communicate any financial concerns with our team proactively
  • Payment arrangements be established for qualifying balances when needed

Our goal is always to work collaboratively with patients while maintaining transparency and consistency in our financial policies.

Cancellation & No-Show Policy

Appointments cancelled same day or without at least 24 hours notice will automatically result in a $50 cancellation fee charged to the card on file. Each additional same-day cancellation or no-show will increase by $50.00 per occurrence.

Patients with 3 consecutive cancellations and/or no-shows may be subject to discharge from the practice due to non-compliance with scheduling policies and interruption of the recommended plan of care.

This policy helps us protect appointment availability for patients actively seeking care and allows our providers to maintain quality scheduling and continuity of care for all patients.

Why do I owe money if I have insurance?

Insurance plans often include deductibles, co-pays, co-insurance, or non-covered services that remain in the patient’s responsibility.

Will I know my cost before treatment begins?

We make every effort to verify benefits and provide estimated patient responsibility prior to treatment. However, final costs are determined by your insurance company once claims are processed.
While our office is happy to assist with benefit verification, it is ultimately the patient’s responsibility to contact their insurance provider directly for detailed information regarding their coverage, deductible, co-insurance, co-pays, and overall financial responsibility.

Do you offer payment arrangements?

Yes. For qualifying balances, payment arrangements may be available. Our team is happy to discuss options when needed.

What if my insurance denies treatment?

If a claim is denied, our office will review the denial and discuss the next steps or alternative options with you.
